Trans-Inclusive and Gender Affirming Care
F5’s trans-inclusive and gender-affirming benefits provide coverage for the treatment of gender dysphoria. Benefits are provided for medically necessary surgical services, including facility and anesthesia fees. Covered treatments include:
- Up to $5,000 travel benefit for covered gender-affirming care

- Gender-affirming surgery: bone reduction/enhancement, breast augmentation, thyroid chondroplasty, rhinoplasty
